Solventum (Formerly 3M)™ advances and streamlines compression therapy with the Coban™ 2 Layer Lite Compression System, a solution designed to help patients manage venous leg ulcers (VLC). Application of standard and Lite systems assists with reducing edema and pain to help patients better get through day-to-day activities. Once applied, the Coban™ 2 Layer Lite Compression System remains in place during wear.

The Solventum (Formerly 3M)™ Coban™ 2 Layer Lite Compression System provide reduced compression (25-30 mmHg) for patients with ABPI greater than or equal to 0.5. Each system is supplied as a kit that includes two rolls:

  • a Comfort Layer roll (4″ x 2.9 yds)
  • a Compression Layer roll (4″ x 5.1 yds)

With compression systems, it is important to monitor the patient’s response to therapy. Wrapping too tightly may impair circulation. Monitor the area of application frequently for signs of discoloration, pain, numbness, tingling, or other changes in sensation and swelling. If these symptoms occur, remove Coban 2 Compression System promptly and contact your local health care provider.

How to Use Solventum (Formerly 3M) Coban 2 Layer Lite Compression System

The Inner Comfort Layer – Apply this layer with the foam side against the skin, using just enough tension to conform to the shape of the leg with minimal overlap. This ensures a breathable and thin application that promotes patient comfort and joint articulation.

  1. Start the application with a circular winding at the base of the toes, beginning at the fifth metatarsal head.
  2. The second circle of winding should come across the top of the foot so that the middle of the bandage width approximately covers the articulating aspect of the ankle joint.
  3. Bring the next winding low, around the back of the heel leaving the plantar aspect of the heel uncovered. 
  4. Proceed up to the knee with minimal overlap, using just enough tension to conform to the shape of the leg. Cut off excess material. Give light pressure at the end of the bandage to ensure that is stays in place during application of the compression layer.

The Outer Compression Layer – The compression layer is designed to be applied at full stretch throughout its application.

  1. Start the application with a circular winding at the base of the toes, beginning at the fifth metatarsal head.
  2. Complete up to three figures of 8 around the ankle ensuring the entire heel is covered with at least two layers.
  3. Proceed up the leg with 50% overlap to cover the entire inner comfort layer. Maintain consistent stretch throughout the bandaging process.
  4. Press lightly on the entire surface of the application to guarantee an optimal comfortability and to ensure that the bandage adheres to itself and to the inner comfort layer.

Removal – The compression system may be removed with bandage scissors or by unwrapping. If using scissors, it is advised to cut on the medial side of the leg and ensure cutting behind the medial malleoli.

Can Solventum (Formerly 3M) Coban 2 Layer Lite Compression System be used for lymphedema?

Yes, the Coban 2 Compression System is indicated for the management of edema associated with lymphedema.

Dermatitis and eczematous changes occur with long-standing edema and extravasation of blood and circulatory components into subcutaneous tissues. Effective edema reduction is the most important treatment to reversing these skin changes, and the Coban 2 Compression System has been clinically proven to reduce edema. There are many skin products used for topical management to
reduce inflammation, moisturize, and protect the skin that are compatible with the Coban 2 Compression System. Solventum (Formerly 3M) recommends the minimal amount of cream or ointment necessary to moisturize the skin.
The Wound, Ostomy, and Continence Nurses Society™ Guideline for Management of Wounds in Patients with Lower-Extremity Venous Disease suggests to:
• Avoid the use of skin irritants and allergens.
• Use emollients such as petrolatum to counteract dryness and scaliness.
• Consider topical steroid ointment as needed for no longer than two weeks.

Coban 2 Compression System absorbs and wicks away skin moisture but is not designed or indicated for use as a primary wound dressing. The wound should be managed with dressings appropriate to the wound condition.

Coban 2 Compression System should be changed if it becomes loose fitting, when it no longer conforms to the shape of the bandaged area, or becomes wet. Avoid excessive heat and humidity. You should contact your local health care professionsal to determine if the dressing should be replaced.
