The patient MUST meet the considerations outlined below and:
- Patient is NOT being seen/treated at home by a medical professional
- Patient is NOT an inpatient at a hospital or skilled nursing facility

Please Read these Considerations before Contacting Us

If the patient is seeking billing services under their Medicare plan for the following supplies, they will need to meet the following conditions:

For Ostomy

Patient must have been seen at least once in the LAST 12 MONTHS by a physician about the Ostomy condition and visit notes are in their patient file
Patient is NOT currently billing for ostomy supplies elsewhere

For Wound Care

Patient must be seeing a physician monthly for a wound assessment
Patient is NOT currently billing for wound care supplies elsewhere

For Urology

Patient must have been seen at least once in the LAST 12 MONTHS by a physician about the urological condition and visit notes are in their patient file
Patient is NOT currently billing for urology care supplies elsewhere

Before contacting us, please confirm and have ready:

The patient has Medicare insurance and the name of any secondary insurance
The type of supplies being prescribed for. If possible, the product name(s) or reference number(s)
The patient has a valid prescription and the date of their last order billed through their insurance
